The situation
In this fictional example, a component manufacturer records completed work in a production log, inspection results in a separate file and dispatch in a third system. A reviewer cannot reliably follow a selected delivery back to its release authorisation.
The investigation
The team follows several jobs, talks through the decisions with the people involved and maps where information changes hands. It discovers that no shared reference is consistently used across the three records.
The practical change
The example team agrees a common job reference and makes the release responsibility explicit. It updates the handover instructions and works through examples with production and dispatch colleagues.
The verification plan
A later review samples completed jobs, checks whether all three records can be connected and confirms whether release authorisation is visible. If exceptions persist, the team revisits its explanation of the cause and the practicality of the change.
