Separate completion from effectiveness
An action can be complete before anyone knows whether it worked. Updating a document or briefing a team records a change, but later evidence is needed to understand its effect.
Agree the review question
For an illustrative dispatch problem, a useful question might be whether later jobs contain a traceable release decision. Choose evidence relevant to the cause that the action was meant to address.
Keep the result connected
Record the action owner, the later sample and the conclusion together. If the same issue persists, revisit the cause and the practicality of the change rather than treating the completed task as the end of the matter.
