Build an evidence register
One question, one traceable record
An evidence register connects a review question with the document that helps answer it. Start with the machine configuration and document revision, then record the evidence location and the owner responsible for keeping it current.
Useful fields
Include a reference number, the engineering question, the evidence record, its revision, the reviewer and the current action. Distinguish “evidence received” from “evidence reviewed”. These are different project states.

Make the gaps visible
A register should help the team find missing links. An open item needs an owner and a next step. A file reference that cannot be retrieved is not a useful evidence link.
