Close a finding with evidence, not an intention.

Build a follow-up record that distinguishes planned work from an observed result.

Keep the original issue in view

An action should address the condition described in the finding. Link the response to that condition so a reviewer can judge whether it resolves the issue rather than merely improving something nearby.

Confirm implementation

A revised procedure may explain the intended fix. Operating records, interviews or a focused test can help establish whether the change has reached practice. Choose a method that fits the issue and state its limits.

Review effectiveness proportionately

Some changes need time before their effect can be observed. Keep implementation and effectiveness conclusions distinct. If follow-up remains open, assign responsibility and a review point rather than marking the work complete prematurely.

Use it in a review

Write down the question, name the record that could answer it and identify the person who can explain its limits. If those three things do not line up, refine the enquiry before gathering more material.
